All applicants must appear in person.

Both parents must accompany children ages 15 and under.

|
One parent is required for 16- and 17-year-old children.
|
|
You will need to know your social security number.

Use BLACK INK ONLY when filling out the applications.

You will need the following:
PROOF OF CITIZENSHIP: - one of the following:
Option 1: A certified Birth Certificate issued by the city, county, or state*
Option 2: Consular Report of Birth Abroad, or Certification of Birth Option
Option 3: A full validity U.S. Passport book or card (can be expired)
Option 4: Original Naturalization Certificate or Certificate of Citizenship
*MINORS UNDER 16 with a previous passport MUST ALSO SUBMIT a certified birth certificate.*
*Minor applicants must be accompanied by both parents.*

PROOF OF IDENTITY: - one of the following:
Option 1: Valid or recently expired US Passport
Option 2: Valid driver's license (not temporary or learner's permit)
Option 3: Naturalization Certificate of Certificate of Citizenship with identifiable photo attached
Option 4: Official US military or military dependent identification card
Option 5: Government employee identification card (federal, state, county or municipal)
Option 6: Current (Valid) foreign passport
Passport Photo:
One 2"x2" passport photo is required. Photo services are available on site for $10.00.
